I hereby authorize Conditioned Air Company, its employees, agents and independent contractors to conduct an investigation into my background for the purpose of evaluating my qualifications for employment. I agree that Conditioned Air Company, its employees, agents and independent contractors may request information from former employers, educational institution, business and professional organizations, credit bureaus, local, state and federal law enforcement agencies, individuals with whom I have been associated and with any others who may have information regarding my competence, character of qualifications and any and all other sources considered appropriate by Conditioned Air Company.


I hereby release Conditioned Air Company, its employees, agents and independent contractor from any liability for their action in investigating, considering and evaluating my competency, character and qualifications. I further release from any liability all individuals and organizations who provide information concerning my competence, character, qualifications and other applicable information for employment with Conditioned Air Company.

I further understand that if employed I will be on a 90-day introductory period, and that termination for unsatisfactory performance during that period will not result in any Company responsibility for unemployment benefits. I further understand that completion of the introductory period does not confer any expectation of continued employment, and that if employed, my employment will be for no definite period and "at-will."

By signing below, I certify that all of the information that I provide on this application and in any interview will be true, complete and accurate in all respects, and I agree that if the information is found to be false, misleading, or unsatisfactory in any respect (in the Company’s judgment) that I will be disqualified from consideration for employment or subject to immediate dismissal if discovered after I am hired.
